Happy Faces is dedicated to reducing waste, and maintaining workflows that embrace technology. By doing so, we are able to control costs, increase our efficiency and provide better service and resources to all of our customers, contractors and employees.
We are implementing a new system that allows us to interact with all of our employees without the need for the plethora of paper based forms customarily used with human resources departments and staffing agencies. Until this system is completed, please use one of the links below for the appropriate form.
The forms are designed to be completed on line without the need for printing. When you have completed the form, you can use the e-mail link on the form itself to submit it directly to us, or you can save it to your computer and use our EMPLOYEE UPLOAD FORM to send it to us. Forms that contain personal information will require a password to open and should be transmitted to us by way of our upload form, (NOT E-MAIL) for added security. If you try to open a form that requires a password, please contact our office for the password.

Direct Deposit Authorization Form
Please complete this form and submit to us electronically. You are able to choose up to 2 accounts to have your paycheck deposited into. We will eventually need a blank voided check, blank voided deposit slip or a letter from your bank indicating you as signature on the account. Copies of these items can be uploaded to us using the EMPLOYEE UPLOAD FORM, you can fax them to our secure E-FAX 1-888-427-7931, or you can bring them to our office for us to scan to your file.
Request For Time Off
Please use this form to request time off. If you are an employee working on assignment you will need to seek the approval of your supervisor prior to submitting . We ask that you complete the employee section of this form and then print it. You will need your assignment supervisor to sign it. Once it is completed, please either upload the document to us using the EMPLOYEE UPLOAD FORM, faxing it to our secure E-FAX, 1-888-427-7931, or bringing it into our office. You will be notified of the approval and any required payroll information via e-mail.
Tax Status
If you wish to change your tax status at any point, please complete these forms. For your convenience, we have combined both the Georgia G-4 withholding statement and the Federal W-4 withholding statement in this single document. The forms can be completed on-line, but must eventually be printed for your signature before submitting. Changes to your tax status may take up to 1 pay cycle to complete, and may not be instant. Be sure to date the forms appropriately, as we will use the most recent dated tax forms to program your tax status.
Timesheet
This is our official timesheet for all work both internal and external. Please be sure to have this document faxed to 770-414-9072 or 1-888-HAPPY-31 (1-888-427-7931). All timesheets need to be received by 5pm on the Monday following the work week.
